Medication errors in hospitals can be prevented, with pharmacists playing a key role in making changes to drug therapy. A study in a South Indian hospital found that pharmacists made 470 interventions over six months, with most being passive interventions. Out of the interventions assessed, 91.30% had beneficial outcomes. This shows that involving clinical pharmacists in patient care can lead to better treatment decisions and improved patient outcomes.
